Visiting Registered Nurses

Skilled Nurses must have clinical nursing knowledge and skills in  physical assessment and procedural teaching and. They are directly responsible for assessing, planning, providing and documenting nursing care in accordance with the physician's plan of care and agency's policies and procedures. They will implement, supervise and coordinate provided services , as well as utilize division and community resources to meet patient’s needs. The will supervise LPNs and Home  Health Aides,  and completes documentation.


DUTIES AND RESPONSIBILITIES

  1. Completes referrals and takes physician’s verbal orders as necessary in a timely manner. Channels referrals appropriately to Director of Nursing for assignment.
  2. Makes home visits, performs physical assessments, evaluates patient's needs and consults with physician if necessary to develop plan of care and documents appropriate and in a timely manner.
  3. Assesses need for additional health or ancillary services and refers patients for service(s) when appropriate, on a timely basis.
  4. Assesses family and home environment and makes recommendations, which will Enhance patient care and promote safety


QUALIFICATIONS:

  • A graduate of an accredited nursing program
  • Have a current Illinois license as a Registered Nurse:
  • Have at least one (1) year clinical nursing experience, preferably in rehabilitation or medical-surgical nursing (home health experience preferred).
  • Have current CPR certification.
  • Possesses interpersonal, communication and organizational skills necessary to complete job responsibilities.
  • Has an insured automobile and a valid driver’s license, or access to adequate transportation.
  • Has the ability to make home visits and provide skilled nursing assessments and skilled care in the home.
  • Must not pose as a "direct threat" to the health or safety of the individual or others in the workplace or in the job performance.

Licensed Practical Nurses

DUTIES AND RESPONSIBILITIES

  • Administers nursing care in a home care environment under the supervision of a registered nurse or other medical supervisor. 
  • Provides basic medical care, including changing bandages, administering medication, and collecting specimens. 
  • Ensures the health, comfort and safety of patients by assisting with bathing, feeding, and dressing. 
  • Monitors and reports changes in patient's condition to supervisor. 

QUALIFICATIONS:

  • A graduate of an approved LPN educational program. 
  • Have a current Illinois license as a Licensed Practical Nurse
  • Have at least one (1) year LPN experience
  • Has an insured automobile and a valid driver's license or access to adequate transportation
  • Has the ability to make home visits
